Initiatives for promoting the safety and quality of healthcare are growing in number worldwide, with the involvement both of institutions' senior management and their staff. But most of times, we don not know haw can do it?How can we promote patient safety?

I think the safety of the patient can be improved if the patient is reassured and given some hope. Giving careful attention to the patient as well as the system would also help.
Promoting patient safety goes hand-in-hand with person-centred care.
I think, unfortunately disease is more important than the patients for medical and nursing system and this problem can prevent patient safety promotion!

What is the most important dimension of patient safety and what is the boarder of nursing role and medical role in patient safety promotion?
Α fair and just error reporting, analysis, and feedback system with training and rewards for patient safety. Educating board members and senior, midlevel, and line managers (nurgsing and medical role)about the link between management practices and safety.
1) Important thing is to create internal reporting system and "safety culture", it means not blaming and punishing the staff for reporting adverse events during health care delivery.
2) There exist a lot of managerial tools that can be implemented, but also controlled/supervised by a responsible person, on the level of organisation. But any HC organization and any HC professional is working within some health system and its limitations.
3) As mentioned in answers above, there exists some tools and strategies focusing common (universal) failures in healtcare delivery. The European Union Network for Patient Safety and Quality of Care and its website was created improve Patient Safety and Quality of Care through sharing of information, experience, and the implementation of good practices, see its website.
4) Promoting/mproving patient safety is a neverending process.

This is a very important question, to which I do not think that there is a 'one fits all' answer. However, certain actions are important everywhere, although not all with great effectiveness.
1. All team members as well as managers have to get involved and feel part of the promotion.
2. A continuous high profile of the importance will help to keep a focus on this problem ie with poster campaigns, education sessions, reminders in meetings as well as behavioural examples.
3. Involving staff (and students) in quality improvement projects including those concerning patient safety to keep this in focus.
4. Arrange regular team briefings on this topic - maybe by a group of people from all areas who arrange this.
I have come to Gaza 3 years ago and have introduced some of these strategies, of which some have been very effective. Since then a change in attitudes has been noticed also among visitors who regularly come to Gaza from Oxford University.

All stakeholders are responsible to see that no harm occurs to patients/clients in our community. Stakeholders include: patients; healthcare professionals, society, administrators, accrediting organisations, legislative bodies, regulators. The above are as well clients. So, we have to do the right NOW!!! for all of us!!!! Client safety is our responsibility!

Another way to improve Patient Safety is using developing Unit Base Teams (UBT) for each department. These small teams look within their areas to improve service and safety.
