Looking for interventions (research/collaborations/projects/initiatives etc.) aiming to involve informal caregivers of frail elders on hospital wards (e.g. at patient, unit or ward-level). What are the main challenges and supporting factors to involving carers in supportive care (information giving, choice, decision making, fundamental care, planning care and managing transitions etc.)?
