I use TMS to evaluate the cortical excitability of subjects with different movement disorders. Recently, I have started acquiring recordings from patients who have rest tremor or dystonia of hand (from FDI muscle). In many cases, the surface EMG recordings are not silent. Should I classify these recording as RMT or AMT? Is there any technique to ensure RMT is measured despite muscle activity?

Navigated TMS with cortical motor excitability brain mapping will help I think .. first navigated so as to have a individualized 3D MRI for each patient so through this you are sure that you are targeting the primary motor area and not just based on the 10-20 international EEG system and the navigated TMS will highlight for you the site of maximum stimulation that appears in numbers as well as positive and negative waves beside this cortical mapping will help you to detect the true site of motor threshold of the desired muscle and through it repetitive stimulation to the same site is precious and repeating with same values taking into consideration the silent period or after discharge will further confirm that the obtained values are the MT and not movement artifacts
