In that case, Dr. Kumar's suggestion is the most efficient way to cite material, using google scholar. When the source is located (in google scholar), under each there is a "cite" hyperlink, clicking on it will display three citation styles. From among them you can choose APA, cut paste etc.
I am about 70-80% sure the citation is awarded to the author.

Now, the problems are numbers 3 and 4. If the title of the book review and the title of item reviewed are the same, should we remove one at the expense of the other or should we write both as follows. It that correct or one of them must be removed.

This citation in APA applies to book reviews where the reviewer has not given a title to his work. (NB. If there has been a doi number it would come after the pages)
