It is usually helpful to put headers for a group of questions in a questionnaire. The title should be simple and understood by potential respondents, so it is not necessary to put exactly PM maturity, a more simple heading can be more suitable.

Think it depends on your objective after considering all angles. The key objective of the questionnaire is to solicit genuine input from respondents. In some questionnaires, you can provide headers to enable respondents understand better your questions to ease their filling up. However, in some questionnaires, you shouldn't put any header (or sometimes you might want to jumble up / combine all variables' questions together) so that you don't color / influence respondents' pre-judgement before they fill up the questionnaire.

Re your detailed questions / comments provided, I prefer not to include header like PM Maturity etc. sensitive header / information that might influence respondents to provide bias / non-genuine answers.

It is also a matter of whether you will be administering the questionnaire yourself or getting it done. If you are not doing it yourself, I would recommend having the headers.

In my opinion, you can give the main title for the entire text.

And divide the whole text into subsections - each having as a title - just a Keyword.

I've developed quite a few questionnaires across a range of areas. I agree with the orher responders. It is your responsibility to ensure that your target audience can understand how to move through the questions and are clear on what you want and how to answer in order to meet your needs. Use the language of the responders, use titles to help them narrow down what each section is for, and if appropriate, structure your quedtions from the general to the soecific or if on levels from the low score to the high score. Appropriate information documentation helps enormously.

I concur with the previous respondents to this question. All things considered, I would advise structuring the questionnaire/survey by clustering items pertaining to the same construct and using a header to identify the focus of the group of items.

For example, in creating a questionnaire for teacher candidates to self-assess the degree to which they reflect the key dispositions identified in the research literature on effective teachers, I would create a section for each disposition category and then group all the items pertaining to that category together. Then go on to another disposition and items, etc.

This is especially important if the questionnaire/survey is long (more than 30 items). Respondents value a structure in longer documents, and tend to not complete the questionnaire/survey if they become frustrated.

The one exception is if the content of the questionnaire/survey is one that is likely to cause respondents to choose socially-acceptable answers or answers that position him/herself positively. In such cases, it is typical to ask items about the construct multiple times throughout the questionnaire/survey worded in different ways and then examining the consistency of the responses.
