All of us know that several Journals have been following different patterns regarding structure of manuscript, referencing and author guidelines. If our manuscript is rejected from one publisher and meanwhile we want to send it to another publisher, then, we have to change the layout and content of the manuscript. Don't you think there should be homogeneity, harmonisation and standardisation in the language, layout, design and referencing of top ranking Journals for minimizing the efforts of authors.

I don't think there is need for homogeneity and harmonisation of the author guidelines as you said. Remember that most journals especially published by Elsevier followed "your article your way" at the initial submission. After acceptance, you will then be directed to make all the necessary formatting of layout, design etc based on the journal guidelines. Even the reference style mostly adopted is APA6th, but with few journals using the numbered referencing.
Dear Bodh R Sharma You have raised a good point. No doubt it consumes a lot of time in case one's article get rejected by one journal. I think there is a need for developing a consensus by the publishers. However, their identity and commercial interest perhaps prevent them from doing so.
Pertinent point. However, a myriad of potential conflicts - of interest and of practicality - make homogeniety a distant dream. :-)
Bodh R Sharma Thank you for raising such an important topic. I also agree with you. Journals should have an universal guideline for the authors. It is really very pathetic to prepare manuscripts for various journals separately.

You pointed to an important direction. May I argue that top publishers are aware of these problems you refer to and scientfic committees do their best to eliminate emerging problems.
