I am charged with conducting the Community Health Needs Assessment (CHNA) for the nonprofit hospital where I work. The CHNA is meant to capture input from community members about their health priorities, and state guidelines strongly encourage a focus on the social determinants of health in addition to health behaviors and outcomes.

I have developed two versions of a survey that could be distributed to the public to gather feedback (I'll also be conducting community meetings, focus groups, and interviews later in the year). The extended version of the survey includes much more detail, but being longer will likely see a lower response rate. The condensed version of the survey provides less detail, but will likely see a higher response rate.

Would folks be willing to look over both versions and offer any feedback you have for improving either one or both? Any advice you can provide would be immensely appreciated.

(Note: I've closely reviewed resources from the CDC, MAPP, The Community Tool Box, Community Commons, and various other sources, including sample surveys from other hospitals and agencies.)
