I'm not expert in the field of supercooling, but I think you can follow this general procedure. In case of supercooling, you need to define opportune boundary conditions between the vial walls and the substance contained (or at the outside, according to your problem). You can define the slope of the PCM by means of the phase diagram. The same applies to the partition function.
In general, you can define these bcs by using for example weak formulations.
