I use to apply the Honey-Alonso test, to determinate the learning styles of the students (active, reflexive, theorical, pragamatic). I don't know if the test is in English, but probably you can fins an equivalent.
You could look at some of the public interest surveys around the globe and see if you can adapt some of their questions. Look up the Eurobarometer Science & Technology reports, and the US reports on Science and Technology: Public Attitudes and Understanding (in the National Science Board's Science & Engineering Indicators reports). The Questionnaire of Opinions on Science, Technology and Society (Spanish acronym COCTS, Vázquez, Manassero, & Acevedo, 2006) is used to assess attitudes and believes about science and technology, could be useful for you and there is an english version (www.oei/COCTS/) .
