I think that, rather than waiting for one 'off the shelf', you'd be better to create one yourself according to what you want to find out; also, you'd probably have to modify an 'off the shelf' template.

Suggest the following to operationalize your construct / variable in your research model / conceptual framework:
Determine the operational definition for all your constructs / variables through rigorous literature review.
During the above literature review, adopt / adapt the respective survey questionnaire used to define and measure the constructs (remember to seek approval from the original author / owner of the survey questionnaire as part of the research ethics)
If there is no relevant survey questionnaire to measure the constructs, borrow from other discipline's related constructs & adapt accordingly as well as ensuring the survey questionnaire is measuring what the operational definition has derived for all the constructs.
If there is no related construct's survey questionnaire to borrow, you might need to develop your own survey questionnaire (once done ensure you conduct a pilot study to test its reliability & ensuring respondents can understand & fill up your survey questionnaire).
Research data journals and literature for surveys similar to what you are looking for as suggested above by previous researcher. Look at the surveys available and if not fit your criteria or hypothises develop your own. By doing this you will create a piece of original work for which you can take credit and look forward to others cite you work when they are searching for something similar. It's good for you as you develop the skills in research and Will provide a benefit to social scientist who seek to enlighten trade s.
