The Ottawa GRS (Ottawa Crisis Resource Management Global Rating Scale) is good, although it might not quite be what you're looking for. It' breaks down into leadership skills, problem solving skills, situational awareness skills, resource utilisation skills, communication skills, and an overall performance. It's tried and tested, and can be used to self assess or instructor assess.

We did some research on disaster preparedness and education of ED nurses and in nursing education. The survey was used with slight adjustments for a similar survey in Australia (stats running) and planned for use with he European association of emergency nurses.
