We are about to embark on a study titled "Participatory health promotional intervention on menstrual hygiene practices among rural and urban high school students" We therefore welcome information about standardized tools fellow researchers have used previously to obtain data on similar topics.
UNICEF had a programme for assessing hygiene. One was "WASH in schools". They had a toolkit for assessment of menstural hygiene management (MHM) practices. I hope it might be useful to you. It might require changes / adaptation to 'local' use, but it must be easy and there has already been large scale data collection done in this regard and the reports can be searched at UNICEF site.

The only problem I see is that the participant does not identify their age. I would guess that you have identified the age grouping to be participants. As you are aware, knowledge and attitude often are based upon experience. In the population I work with, menstrual bleeding can begin any time from 10-16 years. Those who start menstruation early require a different approach than those who start late (and wonder if something is wrong). Even with strong educational efforts in school and with families, I continue to be amazed with some of the myths expressed when these children/adolescents present for counseling in our clinics.
In our populations, I have thought that an effort such as you are taking, would be good to evaluate the school programs in place. But, neither the health care systems nor schools seem to think the time required would be of much assistance in their curriculum development.
