This isn't a review, but here's a community-based intervention programme that I was very proud to be involved in, which is on here: Stephen James Minton, Michelle O' Mahoney, Rose Conway-Walsh - A ‘whole-school/community development’ approach to preventing and countering bullying: the Erris Anti-Bullying Initiative (2009–2011) - Irish Educational Studies 06/2013; 32(2). DOI:10.1080/03323315.2013.784637. I hope that it's of some interest to you.
This is a wonderful topic (Child & Adolescent Mental Health - C&A MH) although, I think, one of great neglect, especially here in the USA. As one with expertise in (C&A MH), I am increasingly dismayed about the lack of knowledge and insight about this population, I"m sure a topic for another more.
Prevention wise, there are various programs aimed at "alcohol and drug abuse," "smoking/tobacco products," and, of course, everyone's favorite dog to kick, "(anti) bullying." In my area, Franklin County, Massachusetts USA, several supposedly evidence based/best practices programs have been initiated, especially in schools, in order to address the above mentioned problem sectors.
Coordinated by the Communities That Care Coalition (CTC), a group of county agencies and personnel work together to implement various programs, using tools such as "Social Norms Marketing" to, hopefully, change the mind-set of the targeted audience(s). This link is for the website: www.communitiesthatcarecoalition.org/.
While, much to my chagrin, the CTC doesn't specifically address C&A MH (they do address tertiary factors) members and staff work hard to improve the overall health of parents and children. Both co-chairs (under the Contact link: http://www.communitiesthatcarecoalition.org/contact) are well known to me so feel free to let them know I directed you to them, assuming you wish to get in touch.
If any of their work is suitable for your purposes, I'm certain they assist you.
I trust this is the sort of community prevention activity that you're seeking.
