If you are in the United States, it depends on the state you are in. There are so many changes taking place in what is required for professional development for teachers. This is beyond requirements for certification or licensure. What information do you want from the questionnaire? That will help in finding what you need.

I am interested in creating an instrument which focuses on cognitive-contructive view of professional develompent. Acctually, developing their personality, interpersonal capacities to work in education, perceptions, views, beliefs, professional selfperception. competencies for reflection.... sort of holistic orientation of teacher, attitude to professional development wich is not linked to any specific professional area of teaching (like mathematic), but it can be used for all teachers (also preschool teacher, and so on).

But to study this - you must also measure teacher motivation and teacher morale - otherwise this will confound the study and further it is very important the kind of assumptions you are going to make with regard to teachers and about teachers.
