I have no questionnaire as such, but can provide a few ideas to set it up :
- How often are you asked to facilitate co-creation workshops ? Per month ?
- How often are you asked to participate in a co-creation workshop, as a stimulator of creativity
- Do you know the term "serendipity" and what it means ?
- How often do you feel like being confronted to serendipity ? Per month ?
- Are you practicing an artistic discipline ? As a creator ? An interpret ? Both ? Level ?
- Have you got one of the following official roles ? Creative workshop facilitator / researcher, designer, ...

I appreciate your help. But I am considering using a maturity scale to measure this variable. Because comparing with setting the scale up on my own, the reliability and validity of the maturity scale are relatively high and I have found such a scale exists. So, I am approaching the authors to get it.
