Randomized clinical trials (RCTs) are the main source to guide physicians in the selection of treatment options. Randomization is the only reliable method to control for confounding factors when comparing the treatment groups. However, RCTs have several limitations (Selected patients, according to a protocol processing, costs of studies, regulatory requirements, delays in approval ...). In addition, RCTs may not represent the practice in the real world. A possible alternative would be to conducting randomized trials in a registry that includes a large cohote (almost the population) of all-comers.
