If a picture (or any other data) was already published, you do need a permission from a publisher. Usually not a problem.

it depends on the publisher (check on their website for policies). For most, authors retain rights on figures, so you only need to get agreement from the authors (but publishers can also give permission). Of course, you should put the proper citation. For data, I think that the authors always retain rights (it is the case in E.U. and in the US for sure).

Nowadays, most of the journals grant you permission online. You just need to fill some data regarding general details of what you are writing.
