I think defunctioning mean totally divert bowel contents from distal bowel segment. While decompression is evacuations of proximol bowel segment in order decrease pressure on bowel wall following distal obstruction

Defunctioning usually means exteriorisation of the colon as a part of surgery (resection) and prior to re-anastomosis, as a way of improving outcome. It does not obligatory exclude Decompression as a result of Defunctioning, but Decompression (evacuation of colonic content, gas and feces) may be achieved by other means. For example, rectal tubing, enema, etc.

defunctioning is diversion of the contents ex.defunctioning loop ileostomy to divert the contents and protect LAR.Decompression is to empty the contents in an obstructed bowel ex.using a savage suction device.

I think you are talking about obstructed cancer rectum.

If this is correct, obstructed cancer rectum is managed by a loop sigmoid colostomy not loop ileostomy. Obstruction by cancer rectum causes distention of the colon and if the ileocecal valve is competent (i.e. prevents retrograde passage of colonic contents into the ileum i.e. prevents decompression of the obstructed colon into the ileum) this can produce a closed loop obstruction of the colon i.e. the colon is obstructed distally by the cancer rectum and proximally by the competent ileocecal valve; in this case the colon will distend progressively until it perforates. The management of obstructed cancer rectum is decompression stoma i.e. a stoma that will deflate the colon; a loop sigmoid colostomy will do that; a loop ileostomy will deflate the colon only if the ileocecal valve is incompetent allowing reflux of colonic contents into the ileum then through the ileostomy; if the ileocecal valve is competent loop ileostomy will fail to decompress the colon.

If properly constructed a loop ileostomy or a loop colostomy defunction the distal bowel i.e. prevents stool from reaching the distal bowel. Most commonly this is needed in case of low colorectal anastomosis after excision of cancer rectum to protect the anastomosis from stool until it heals.

I meant that a loop ileostomy is done after Low anterior resection or after colo-anal anastomosis to protect the anastamosis in healing by defunctioning the colon.For obstructed sigmoid colon , Hatrman's resection with End colostomy is the usual procedure to decompress the colon.

my understanding is that double barrell colostomy done wen resection of bowel is done and decompression is done instead of defunction...

Loop colostomy is done wen posterior mesenteric side of bowel is healthy and no portion of bowel is resected and strict defunction is not required

100% defunction is achieved wen both proximal & distal stomas r made separately at a distance or distal stoma is closed as hartman so that none of gut contents spill over to distal bowel (100%defunction)

Loop colostomy or double barrel colostomy are decompression as they don't 100%defunction as bowel contents can spill over to distal bowel

Distal stomas r usually mucous fistula as they secrete small amount of mucous

Loop colostomy or iliostomy r not in true sense 100% defunction stomas as some of bowel contents can spill over into distal bowel
