i research for a quesstionnaire which could measure the impact of incentives on medical staff performance during the covid-19.

Determining the nature of incentive will help in narrow down the search for the questionnaire. Please see what are the different types of incentives that have influence on intended population.
You should design a Google Form for the questions, and we try to share it in our scientific groups, such as telegram or WhatsApp groups, or here in the researchgate .
