Doctor is an important question although community medicine is dealing with many cases of psychological, family and nutrition, but attention is only in the specialty and the reason is due to lack of information on the importance of community medicine
Does the country(ies) you look at have a predominantly private-led health insurance system? Because this system does not usually consider health issues for communities as a whole. It’s an individual system
Community medicine (alternatively called population health) has historically been ignored by the healthcare providers in the developed countries even more than the developing countries. Historically (and predominantly even now) healthcare system was profit driven, meaning that the basic model was curative care rather than preventive care. Preventative care being a primary model for population health approach was seen by large healthcare industrial complexes as the barriers to profiteering, which was the primary motive for healthcare industry.
More recently, there is a push in developed countries for integration of the two approaches.
Now in developing countries, separation of healthcare and community health was less of a case given that the healthcare systems are primarily owned by the government and community health is also a government function. Also, clinical care is also provided by many of the same doctors who work at the government hospitals.
So I hope that I understood your question correctly but healthcare is often the big/rich sister and community medicine/population health is poor/small sister, which primarily focus on preventive care and clinical services for the poor and underserved.
The previous answers do seem to relate to population medicine vs. primary care that is focused on preventive as well as curative and palliative approaches and also is delivered by persons, often working in health care systems, that are aware of the overall issues of the population they serve. In the US and apparently in some other countries, community medicine is not synonymous with population medicine.
The Community Medicine has to play an important role in developing Countries but unfortunately it is not happening, In most of the places it is theoretical teaching only and the practical exposure with the health system & community is lacking, But there are few good examples also.
the matter is not related to the canals of health care provide. but to the medical planning and determine the needs and the ways to solve health problems.
I agree with you that this is the situation on the ground. and that's why the study of community medicine is less needed compared to surgical and clinical branches. but the real problem is the low governmental attention and appreciation about the role of this branch as a health leader.
The community medicine is known by various names, as indicated in my previous post, and there is a wide variation in practice. Here is an excerpt from a blog that explains it (the complete blog is at: https://blogs.timesofindia.indiatimes.com/global-health-focus/why-community-medicine-public-health-is-the-sexiest-profession-of-the-21st-century/ )
"Community Medicine is understood by different names today (Social Medicine, Preventive Medicine, Public Health, Community Health) and perhaps suffers an existential crisis thanks to regulatory agencies that have never really understood the branch and have not gone beyond dry textbooks and sundry lectures. The Medical Council of India seems to take forever to understand that Community Medicine is a clinical branch and barely able to comprehend the field. "
the name is immaterial and creates confusion only. basically the discipline is involves in promoting community health or public health practices. we have mandate to discharge our social responsibility in partnership with health system and community
You can find examples of good practices in department of Community Medicine at Mahatma Gandhi Institute of Medical Sciences, Sewagram, India. you can visit our website www.mgims.ac.in
in case some specific issues that I can share my publications by e mail also
In our countries the priority attention of health system directed toward curative medicine rather than preventive medicine, and even health professionals and general public do not know the meaning of community medicine .
