Does anyone know where I could get hold of the USA English language version of the CFQ-R Cystic Fibrosis quality of life questionnaire for adults please? I've tried e-mailing the author at the address given on the web but it's bounced back! Any tips would be appreciated.

Also, has anyone used a short-form of this questionnaire or focused on a specific domain rather than administering the whole questionnaire and did this give you useful information?
