I didn’t find a clear way or reference on how to write the discussion section a rare case report.
The discussion section serves to summarize and interpret the key findings of the case report, to contrast the case
report with what is already known in the literature and justify its uniqueness, to derive new knowledge and applicability to practice, and to draw clinically useful conclusions.
In comparing the new case with prior
knowledge, the author should briefly summarize the published literature and show in what aspect the present case
differs from those previously published, and thus deserves to be read and published.
It must;
1. Summarize the essential features and compare the case
report with the literature.
2. Explain the rationale for reporting the case.
3. State the lessons/experiences that may be learnt from the case
report, and how things can be managed differently in
a similar situation/case.
4. State any limitations of the case and the significance of each limitation. The value that the case adds to the current literature should be highlighted, so should the lessons that may be learnt from the
case presented, especially if new recommendations for patient diagnosis (with use of an imaging modality) or management, could be put forward.

Even though references will be limited, but a detailed literature search should be done in all rare cases. If any similar case found, a comparative table can increase the academic impact.
If multiple studies found, a systematic review of the reported Cases in a table can be made
Discussion - is a debate of your findings ( Your case) with similar in literature.
Debate your patient- demographics, way of presentation, diagnosis, investigations and management plan

Rare case report is descriptive type of study, there is no need of make a hypothesis which is required in Analytical study. Before writing discussion section, we can go thorough search review of literature of similar case , you may find similar case report with some differences you can quote the reference. It might be that your case is so unique that you not able to find out any reference . you can make out solid inference from your case for future reference
It is parcel of research structure where a researcher depicts key arguments from the mere data versus literature and or existing reality . It helps us a skipping pave to reach in to the finding. In descriptive social science studies it usually is placed in analysis and presentation section.
In discussion part of case report, Briefly write about the uniqueness of your cases and references available to related case . What innovative idea you have putted to manage the unique case that is different from routine case to get maximum benefit from your innovation
