It is up to you. You determine the research question and the study inclusion/exclusion criteria for the systematic review. Remember, for whatever you decide, you must be able to justify it. Think "would an expert agree with your decision?".

That is compeletly right, but since the derivative have only one RCT! It will be logical not to include into meta-analysis
