Cost-effectiveness of intervention is estimated by dividing cost of intervention to health outcome. DALY, cases averted, deaths averted etc. are commonly used by studies as health outcome. Can incidence rate be used as health outcome of intervention or how it can be converted to commonly used health outcomes?

Typically, costs and effectiveness are presented as numbers (instead of rates) in an defined population/cohort.
It seems OK to me if you would present costs as well as incidents as rates but would produce the same ratio as when you would presented results as numbers (instead of rates) in an defined population/cohort.

I want to present cost in monetary value (number). The aim of project is to evaluate the effectiveness of intervention in reducing malaria incidence (cases). So, can incidence rate be compared between intervention and control arms (using similar estimation by survey) or it should be converted to number of cases averted (though I don't know how to convert, another concern).

Perhaps I am misunderstanding but:
If your cost-effectiveness analysis represents a randomized trial, then it should be relatively straightforward to compare the costs incurred and the numbers of cases in the population of either arm of the trial.
If the trial arms have the same size, you can directly subtract the numbers in the control arm from those in the intervention arm. Then there is no need to calculate an incidence rate first, let alone converting the incidence rate to something else.
If I am misunderstanding, please provide further explanation, considering that I (and most other readers here) are entirely unfamiliar with your study.

Your suggestion simplify things but although we estimated equal sample size, response rate may introduce minor variations.
