The model depends on what you want to study (cyclic behavior, push-over, 2D or 3D). You have the website of Cast3m which gives codes and examples.
Modeling of plain concrete by CDPM requires some data in tabling format as follow:
1- choose the suitable constitutive relationships (stress-strain curve ) for compression and tension behavior of concrete. assume values of strain from zero to ultimate strain and find the corresponding values of stress.
2- calculate values if inelastic strain. inelastic strain = total strain - elastic strain
elastic strain= stress/ initial modulus of elasticity
3- damage parameters (dc and dt ) can be determined from the stress ratio of the descending part of the stress-strain curve.
dc or dt =1-(stress after ultimate strength)/ ultimate strength)
note / can be modeling the plain concrete by plasticity model if dc and dt equal zero.

What kind of laoding will you study? The quality of the results depends on the solicitation. The models are not efficient in every circumstance. Once we know better what kind of loading you want to study, we will be more able to help you.
