Thanks Beatrice!  I’ve come across several of these; unfortunately they’re largely looking at organizational level integration or they’re theoretical in nature.  I’m finding very little clinical/practice level measures for providers....will certainly take a another look at your recommendations though!

Integrated transitional care: patient, informal caregiver and health care provider perspectives on care transitions for older persons with hip fracture

Methods: A qualitative, focused ethnographic approach was used to guide data collection and analysis. Patients, their informal caregivers and health care providers were interviewed and observed at each care transition. A total of 45 individual interviews were conducted. Interview transcripts and field notes were coded and analysed to uncover emerging themes in the data.

Results: Four factors related to poorly integrated transitional care were identified: confusion with communication about care, unclear roles and responsibilities, diluted personal ownership over care, and role strain due to system constraints.

There are only a few.  A relatively common instrument for this category is the SF-12v2, a derivative of the SF-36.  It has favorable reviews but is licensed only through QualityMetrics.  There is a Health Quality Questionnaire through Pearson Assessments that is a version of the SF-36.  In community medical practices, the SF-12v2 is favored partly due to brevity.

I dont't want to blame you, but I do not see the relevance of your recommendation according to Katherine's question. O.k., the SF-12v2 or theSF-36 as a measure of functional health and well-being from the patient’s point of view is a measure of so called patient reported outcomes. But Katherine asked for providers perspective not for patients perspective. I think, that a questionnaire on health retalted quality of life does not adress Katherines question on factors concerning integrated / collaborative health care models.
