As someone who has written many articles that were published in medical journals, I can tell you that pobably no reputable medical journal would publish such a tale. You typically have to be a credentialed medical expert or be represented by one who would agree to lend his/her name to the paper.
But if you're really determined and you're not familiar with medical journals, go to PubMed. Look at the different journals that include case studies (most don't). Narrow it down to three or so that you'd like to target. Then go to the website of the first journal and find the “instructions for authors.”
These instructions include information about margins, headers, figures, font size, word limits, etc. if you don't follow them exactly, it will automatically be rejected.
When you think you have the paper as you want it, follow the very elaborate instructions as to how to submit it through the journal’s web portal.
