I have a 15 year old patient who had a concussion 2 years ago. Her parents report that she is unable to recall past events, As an illustration, they said in the past summer she spent a week at the shore with her friends. With coaching she can remember the event but can recall no details of the vacation. They claim she never had this problem before the concussion.

This condition is not very unusual post head injury or trauma. In our research center we come across many cases and they regain the short term memory loss in fairly short period of time.
One of the key factor that is the conventional paradigm changer and that is as following:
Sub-Nucleus Neuro-Invasive Viral RNA Proliferation develops very rapidly that influences the concerned controlled tissues/ organs and system.
Short Term Memory loss is also one of the functions that becomes compromised until one of the 2 conditions prevail.
1. Increase in the domestic Immunity or
2. Regression of the Viral RNA load at sub-Nucleus.
If not treated, many other adjacent neurons will also catch up to the infection and the respective systems will begin to begin to deter their homeostasis.
Therefore, Run a lab on the Viral Serology of CMV & HSV, Both the IgG & IgM.
Even if the IgG is more than the 10% of the normal cut-off value, treat it with the respective antiviral therapy. You will be amazed with the prognosis positive.

From the clinical vignette, I would say it is more a memory-access problem than a memory-encoding problem. Frontal dysfunction assimilate to memory-access can occur after head trauma but looks pretty severe in this case. How severe was the trauma itself? I assume MRI is normal; EEG is also normal? The patient is young, I would start a cognitive-rehabilitation program as soon as possible.

I would propose a thorough neuropsychiatric and neuropsychological examination, after 3 years of the event. then you could tell with accuracy.

I worked in a multidisciplinary concussion clinic for children and adolescents during my residency at John's Hopkins School of Medicine and the Kennedy Krieger Institute. This is a very atypical presentation for concussion. She needs to be referred for a comprehensive neuropsychological evaluation with a clinician familiar with concussion. Subjective reports of anterograde memory impairments following mild TBI can be associated with a number of underlying factors (many of which are not actually the direct result of the brain injury, but related to the interaction of psychosocial influences) and should be thoroughly investigated in order to identify an appropriate intervention strategy.
