Quinolones should not be the first-line of treatment for UTIs. First line of Tx for UTI should be nitrofurantoin 100mg po bid 5d or TMP/SMX DS 1tab po bid 3d. If you have to use cipro due to allergy to the previous two or local resistance of 15% to TMP/SMX, use cipro 250mg po bid 3d. This is only for treatment of uncomplicated UTIs per IDSA (2010) guidelines. Likewise with enteric infections, you want to withhold quinolones unless absolutely necessary. Enterobacteriacae has a high rate of resistance to cephalosporins. Use a narrow spectrum and the shortest effective dose for treatment of infections.
I already know these guidelines, but I asked this question because I've seen several prescriptions combining quinolones and 3rd generation cephalosporines in treatment of UTI.
Sorry, I guess I misunderstood. I don't believe that it is necessary two antibiotics for treatment of complicated or uncomplicated UTI. That seems like an overuse of antibiotics.
