We are in the transition from rehabilitation ward to treatment and recovery, to promote service user involvement this would go some way but not sure if this has been tried elsewhere.
Nice idea. I have not heard of any. Hope you guys will try it out and let us know. But would their own notes be reviewed with them, or its use affect care? I assume detailed psychiatric notes already include clients views and other related material

Yes Bawo, their notes are already written with a good deal of collaboration and contain patient views but we are interested in a much greater patient involvement in writing their own notes with staff support and monitoring.
