I think you mean testing paired binary data in SPSS. The proper test is the McNemar test. The path is

Analyze--->descriptive statistics --->crosstabs

After that, move "before treatment" to be the row variable and "after treatment " to be the column.

Finally, click on 'statistics' button and choose McNemar.

Thanks Dr. Nadiah for your answer. It is correct to use Chi square in case of both Dependent and Independent are categorical variables. However, in my case I want to test the outcomes before and after treatment so that the case here is quite different. Therefore, McNemar test is the best to be used.
