Is there any reason why you have not considered measures like the MMPI or PAI?

The Symptom Checklist-90 items of Derogatis et al. is a good choice as screening self-report.

p.s. Your question came up under psychopathy, but upon closer read, it sounds like you're looking for something broader than what I'd initially suggested below (TriPM). In that case, a couple that might be useful in addition to those others have mentioned are the Patient Health Questionnaire (PHQ), and PDQ-4 for personality disorder symptoms.
