Why Single QC level (QCL) is out of range repeatedly in the freshly prepared standard and quality control?

We developed a multiplex assay in Dried Blood Spot (DBS). The standard curve keeps on failing. I have been experiencing this again and again:

- When I spike fresh standards and quality controls and check the standard curve, QCL exclusively is always out of the range

- Then, when Re-spike the QCL alone from the same working solution and recheck the curve, all standards, and quality controls are in the range.

What I do not understand is, why the freshly prepared QCL keeps on failing?
