I am working on hypertensive patients. I used to collect their serum samples in gel & clot activator vacutainer. but in two of my clotted whole blood samples (total are 100) when i separated serum by centrifugation at 3500 rpm for 10-15 min. the separated serum was yellow but cloudy and clotted. then on further centrifugation yellow clear liquid separated out and extra clotted yellow layer settled down.
Can anyone tell me the reason why serum was clotted after separation?
if only two have this sort of issue then that's an error either in your side or manufacturer side. Gel and Clot activator may be in low quantity or not thoroughly mixed

The manufacturer recommended tube with clot activator and gel need a minimun 30 minutes for clot time because short time fibrin formation see yur picture (additional clotted material).
But this problem only 2 to 100, and other possible explication is this patient have coagulation problem or take medication (influence a coagulation time) and this tube clot time more 30 minutes.
Have you a coagulation test for this patients?

Yes this could be the reason as these two patients were taking heparin and aspirins thats why their blood was not properly clotted in given 30 minutes.

The cloudiness of the serum in your photo is not normal. It suggests that the patient has abnormally high lipid content.
Lipemic serum can interfere with the accuracy of test results. This is particularly a problem for tests that rely on spectrophotometric measurement as the turbidity of the lipemic serum can give falsely high results.
If I am correct and it is high lipid in your two patient samples, you will need to consider treating the samples to remove the excess lipid before you do any tests.
