Yes, OCT should be a better option. I still cannot understand why people do FFA for every case of uveitis ? Capillary drop-out and neovascularisation is so unusual after uveitis.
Inflammation of the disc, non perfusion, vasculitis and peripheral hyperflorescence just to name a few. The patterns that appear can help diagnose the potential source of the uveitis. Such as Lupus, Sarcoid, TB, Lyme Dz and so forth. Angiography is so essential for uveitis. Most often so much is lurking in the periphery retina that will never be seen on OCT. Sweeps or wide-angle are a must.
Dear Dr Meryem, thank you for your input. I think you must be seeing quite a lot of Behcet patients in Turkey. What are the characteristic features (clinically)? Here we hardly see any of those cases.
after attacks of a retinitis, vitritis or vasculitis in behçet patients, while fundus examination is normal, peripheral vascular leakage is continue in angiography,
ı think that angiography not only use to diagnosis of üveitis, wathcing closely treatment
in case of Papilitis it could be a clue to investigate especial disease such as sarcoidosis, TB, MS, ..... and Also you may consider CNS investigation and systemic steroid!
in case of CME you should consider steroid injection!
vasculitis is also a clue..... even phelebitis or artheritis may be clue to underlying disease!
A diagnosis of papillitis does not require FFA. The diagnosis is clinical and subsequently we need imaging studies to find out the cause. CME is better diagnosed and followed-up with OCT. We can assess the change in macular thickness to give us an idea of response to treatment, something not possible with FFA.
For an instance in Retinitis pigmentosa one may see macular edema and also vitreous cell in this case FFA helps to rull out a simultaneous uveitis! When OCT could not differentiate these.....leakage in FFA are also helpfull to follow the treatments effect as may reduce leakage....
Good discussion. I agree with Dr Marianne regarding indication for OCT, but FFA is useful in various uveitic conditions and at various stages as described in books if not all.
