Although it is understood that, oral lesions related to cytomegalovirus are documented in HIV infections. How would one diagnose Cytomegalovirus at the clinical level in the absence of laboratory diagnosis.?

I don't believe you can diagnose CMV without laboratory confirmation. The only way to answer would be "suspected CMV oral lesions."

The Oral HIV/AIDS Research Alliance (OHARA) protocols have shared end points, which include oral mucosal diseases known to be associated with HIV/AIDS. The OHARA epidemiology/clinical team has updated existing diagnostic criteria of the oral manifestations of HIV including Cytomegalovirus. the OHARA epidemiology/clinical team has developed extensive training modules, based on slide and video presentations adapted for distance learning, as well as training manuals.
