We are working on a care integration project between two non-profit organizations and our local public health department. Reimbursements come from both private agencies as well as public insurance providers. We are interested in models or frameworks others are using or have used to (1) structure and/or (2) evaluate integration. Our focus is on process and structure.
We are aware of the SAMSHA care integration model. We are interested in other frameworks and/or how SAMSHA may be used in combination with organization-level models stemming from organization theory (OT) and/or organization behavior (OB) perspectives.
This depends in part on the nature of the issue for which care is to be provided. If it is directed to a specific infectious disease the answer may be different from those relating to chronic conditions. And if the problem is related to environmental concerns, a third answer would be offered. I will note, however, that the issue of local health focuses with infectious conditions whose import extends beyond local health borders raises a series of problems I'm now beginning to address as air, car, and cargo transports provide increasingly rigorous venues for regional importation.

Thank you all -- Looking into the links you provided. I have also received information regarding Australian experiences in this area. Please note that our focus will be on organization theory and organization behavior. As such, we will draw heavily from these fields for our theoretical background, particularly frameworks or models that align with organization-level strategy in health services integration. We may have to create something ourselves...but this information is helpful.

After a thorough literature review, I decided to use the Chronic Care Model as a target model to be modified according to the organization‘s context. To align disparate aspects of this clinic’s structure, we will leverage the Collaborative Care Model as a process tool. We are using the SAMHSA framework to assess levels of integration throughout.
