b) have a look at Steven Lehotay's work I would be surprised if he did not investigate this - at least in brief.
c) depends on your matrix and the amount of matrix needed/used
d) for a targeted SRM method I would initially bet my money on removal plates when its plasma samples, simply because of the ease of use, but be careful with recovery issues.
QuEChERS became a standard sample preparation procedure for food testing, especially for pesticide residues in fruit and vegetable. Many variations of this procedure exist. However, the generic acetonitrile extraction followed by dispersive SPE cleanup is very useful to extract many food residues and contaminants from all kind of samples. The final extract should be diluted to reduce potential matrix effects.
