Amikacin plus meropenem, may be used for initial therapy in serious disease (clarithromycin, doxycycline, trimethoprim-sulfamethoxazole or ciprofloxacin are oral options according to in vitro susceptibility testing)
Surgical debridement and catheter removal also should be consider.
Well, i will explain you. I work in a Laboratory of Mycobacyterias. We receive cultures to do suscebtibility test by proportion methods to TB drugs, Includes first line drugs: INH, RFM, SM. EMB. PZA, and second line drugs: Cycloserine, Levofloxacyn, Kanamycin.
Sometimes we have no many information of patient so, we do directly the Susceptibility test. we check at 7th day. If we have growth it is typed to species level.
We have a case where the patient reports resistance to those drugs i mentioned in the last message.
I havent seen before M. fortuitum resistance to Cycloserine, i wanna know if it is possible that Fortuitum has natural resistance to the drug, when the patient havent been exposed to the drug and has no relapseor any background of other mycobacteria.
