We typically use lateral thigh. Has the advantage of being an easy graft to take, but the disadvantage that it is not always cosmetically acceptable, and not a great colour match for the head and neck area.
Agree. If burn wound is directly posterior (back or legs), then posterior opposite thigh is also easily accessible. Another consideration for unusual location wounds is ease of access of donor site depending on the position of patient on the O.R. table.
Full-thickness grafts from the groin are much more pliable later on, even if darker, too. If cosmetically minor split-thickness grafts (because of shrinkage) are in discussion, then take it in girls or young women from the buttocks within the bikini area.
For small to medium burn wound, the most favourite site to harvest split thickness graft is INNER SIDE of the thigh.
The texture and color match to recipient site is usually not a problem, Technically convenient to harvest graft from this side. Over all in case if donor site wound get pigmented even than it out of sight.
I occasionally use the scalp if the head has been shaved already and other donor sites are sparse. It provides a good source of small grafts and a quick-healing, inconspicuous donor site.
Scalp as a donor site is best for small grafts on the face. It is inconspicuous donor site, good color match to face and quickly healing. The pain is insignificant as well. For other than face we usually use inner thigh or buttock.
In girls and women, we use the buttocks - of course - inside the borders of a bathing suit. However, avoid split thickness grafts as often as possible : fullthickness grafts from the groin or from the whole lower abdomen are much more pliable later on, especially in the face. Their only advantage is also hyperpigmentation - but much less than sprlit thickness grafts !
