I think you would have to extract total DNA from the tissue where the bacteria are, then your primers would amplify only the bacteria, provided they are specific enough.

Would using a pathogen lysis extraction to only obtain the DNA from the bacteria work?

Not sure. I don't work with pathogens, so I don't know what the difference to an ordinary extraction kit is.
