What's the wrong with the deriviation process ?

Hey, Guys.

I am trying to add a diffusion term to the Darcy module of Comsol. It needs the derivation of the weak form of diffusion term. I tired to derive it（the attachement is my deriaviation） but I failed. Its results does not match well with experimental results. could any one tell me any idea why and how can I fix it?
