I believe I did it once for a dental extraction who was experiencing already neuropathic pain in that (dental) quadrant. Despite this, the pain escalated after a couple of months. Unsure whether it was the dental extraction.
I highly advise the use of Pregabalin/Gabapenting a week prior to the dental procedure (especially for root canal therapy and or extractions) in patients already suffering from facial neuropathic pain. Unfortunately, there is no literature supporting the pre- emptive analgesia in these type of patients.
Would you be suggesting the use of pre operative gabapentinoids in any patient or in those surgical interventions with a higher likelihood of developing neuropathic type of pain? I look forward to your comments. Ivonne
Thanks Ivonne, I use it mostly for the outpatients to be able to decrease the
amount of narcotics used in recovery and speed up the discharge process which has been successful. Theoretically it can be used as preemptive analgesic for any patient population with expected pain of moderate to severe.
