Regarding your first question: what you are asking is ideal but, for most (if not all) countries, not realistic. Financing periodic serologic monitoring is another economic issue, on top of addressing demands for diagnosis/detection and vaccination. Serologic monitoring for vaccine response may be more practically-implemented as an epidemiologic (sample-based) survey to assess whether the vaccine induces relevant immune response, rather than on a per-recipient/patient basis (i.e., every vaccine recipient being ordered to get the "follow-up" tests that you mention). Ultimately, receiving a vaccine product does not mean cessation of non-specific precautions against COVID-19 spread.
Regarding your second question: remember the being considered "COVID-19 positive" depends on the test. If some who received the vaccine did not show symptoms after and develops (hopefully) protective immunity by antibodies, anti-SARS-CoV-2 antibody detection on this patient will identify them as SARS-CoV-2 IgG-positive. Identifying the vaccine recipient as "asymptomatic COVID-19 positive" is misleading since they weren't infected by the wild-type virus, they simply received a vaccine product. Furthermore, asymptomatic COVID-19 positive patients are likely able to transmit the virus to surrounding contacts (hence they remain a public health concern)... whereas vaccinated people won't be able to do so (since they weren't infected, to begin with). The COVID-19 vaccine product (even considering the live attenuated form, which hopefully does not become virulent) is not the same as the wild-type SARS-CoV-2.
Hi, tthere is an important distinction here:
after vaccination, recipients without prior contact with the virus will (hopefully) be serologically POSITIVE for SARS-CoV-2 spike protein, but negative for covid-19
if vaccine recipients become infected after successful vaccination, they will not come down with disease (hopefully) and be serologically POSITIVE for SARS-CoV-2 spike protein AND covid-19.
new serological assays should be developed that can discriminate between vaccinated and infected patients. this is done for HBV...
vaccine protects from the manifestation of disease following infection, it does not prevent infection. for this reason, vaccine recipients can still become infected and transmit to others, only they will be asymptomatic.
