What is the starting point for calculating survival (OS & DFS)? Date of initial presentation in clinic or date of completion of treatment?

I have to calculate survival for my study on patients treated for rectal cancer. What is the starting point for calculating survival. Should I take the date of first presentation in clinic or the date of completion of treatment as a starting point??? Similarly for DFS is it starting from the date of completion of treatment?
