Hypertrophic cardiomyopathy (HCM) :Previously known as: Hypertrophic obstructive cardiomyopathy is a primary disease of the myocardium (the muscle of the heart) in which a portion of the myocardium is hypertrophied (thickened) without any obvious cause, creating functional impairment of the cardiac muscle.
Propanolol is a beta blocker and reduces the force of contraction of cardiac muscles. In HCM there is a dynamic gradient between the left ventricle and aorta. The use of propanolol will help reduce this gradient and relieve symptoms.

I fully agree with previous posts. My question in the question is: are more effective selective novelo beta-blockers for caridovacular diseases than old molecules such as propanolol?
well, propranolol is probably most used and with the best evidence, especially in paediatric HCM patients (see Östman-Smith I. Beta-Blockers in Pediatric Hypertrophic Cardiomyopathies. Rev Recent Clin Trials. 2014;9(2):82-5.). However, beta-blockade is not expected to decrease resting gradients; its effect rests on decreasing the rise in gradient that accompanies exercise. For patients who fail beta-blockade the addition of oral disopyramide in adequate dose often will decrease resting gradients and offer meaningful relief of symptoms.

As a personal opinioin please let me say that propranolol is so far the most reliable beta-blocker. This molecule can produce all the pharmacological effects of beta-blockers in a balanced way, it can be effective even in newborns and it can be easily managed both by patients and patients' parents in case of pediatric utilization.
