AP window is a rare congenital heart disease, and 50% had another congenital heart disease, the C.P depends upon the size of the defect, usually this patients had large defect, which make them symptomatic since birth, surgical intervention is the best option for treatment, also can be closed intervention , but on the other hand for the patients with who had small defect , and no body diagnose them,they will be at the risk of development of PVR , and may be became in operable if they developed Eisenmenger complex, but if the PVR is high to the level where the closure be risky for development of acute right sited heart failure, the patient can be started on pulmonary vasodilator, with adjustable closure of AP window my be reasonable, otherwise heart lung transplant
