In the old days we used to just write it in things visual basic, but could use others. If you are wanting ones actually designed for presentation, given I see you are in cog neuro, you might like https://www.u.arizona.edu/~kforster/dmdx/dmdx.htm . However, if you are just doing a standard qualtrics/survey monkey/etc survey, I am surprised one of these doesn't have a save to an executable ability. Have you checked with them?
