I think the best approach to a Becker’s nevus is to use a hair reduction laser along with either a Q-switched laser to lighten the lesion or a fractionated ablative laser.
Beckers nevus has hairs that should be treated first with a long pulsed Nd-YAG laser followed by 1 pass of 532 nm 1-2 J/s 4 mm spot size, which reduces the epidermal pigment that is pronounced in Beckers nevus followed by 1064 nm 4 mm spot size 10-13 J/s till bleeding points for best results.What do you think?

Deep removal of nevus, leads to scarring.
presence of hair, of high density and depth of 4 mm makes th hair removal difficult.
androgenic influence on hair is also a reason for failure.
malonocyte cell concentration being high reesponds very slowly to laser of long pulse. may be the best method will be mohs technique of removal with skin graft
Difficult to treat. At its best a marginal improvement only, in my experience. I have been using 1064 nm. Few patients have undergone repeated sittings for as long as 24 months at sittings ranging from 4 to 8 weeks. No scarring has been observed.
At present I am treating a male patient with large shoulder Becker's melanosis for both hair removal , I use ND-YAG, and for pigmentation , i use Broad Band Light IPL , in this sequence during the same session. He had 4 treatments so far and is showing good progress. His back ground skin is type 4 and the melanosis is type 6.
Dr Wedad, the depth for pigmentation removal may cause disfigurment. Using both IPL and Ng Yag in the same session may not be adivisable in the long run. High andrgenic influence on hair may not help hair reduction. plase try any local anti andrgenetic preparations also. it may be of help.
