Can a 2yr old male with dystonic cerebral palsy develop into a near functioning individual? What are the chances of him to survive until age 10 and still be functional even for just self help skills and reach a communication level of a grade 1 student?
First of all, the diagnosis of dyscinetic cerebral palsy has to be confirmed. Is the a history of severe hypoxia/asphyxia?, hyperbilirubinaemia? If not, another cause for dystonia has to be discussed.
Secondly, it is necessary to do a detailed functional evaluation of the child.n His prognosis is related to GMFCS, to the posibility to use intrathecal baclophen pomp.
His communication has to be developed by introducing alternative and augmentative communication. To the development of his independance, it is very im[portant to start using good and well adjusted chairs and other technical aids.
Thank you for replying Audrone. He was prematurely delivered during hypoxia, thus he has CNS insult. He had BoBath and pt therapies for a yr but he can only turn on his stomach (therapies are too backward in India). For the past two years he cannot sit, crawl nor stand, communication skills are severely delayed but he positively responds thru facial expressions and cooing to his parents or sis when he is talked with. Now his parents, my cousin, is looking for other therapies to rehabilitate his CNS and hopefully Myelination of his system may be jump started. His dad has genetically acquired diabetes with Graves' disease. =(
firstly i must make it clear that a complete status of his reflex development,motor,sensory,neurological ,has to be checked for selection of proper and developmentally appropriate neurophysiological techniques.
The techniques you mentioned does not suffice as what is important is the methodology and not just the names.Like you mentioned that bobath/NDT was applied along with pT procedures.Here i would tell you that NDT has been found to be effective for spastics ,hypotonics but for dyskinetic form of CP only NFDR technique ,which is the latest technique (for improving coordination as well) is most effective.By applying NFDR the dyskinetic patients develops core stability which is the critical problem in thes types but as it is highly specialised so its certification is required
thirdly i must tell you that this latest technique is being developed in INDIA........

Yes, the brain is quite plastic, so it is certainly possible. The chances depend on the CNS damage location and timing, and the intensity of activity-based treatment. See Novak, I et al. 2013 and 2014 for a review of such approaches.
